Diabetic neuropathy deserves special attention in the UAE context, where diabetes prevalence is among the highest in the world. The mechanism involves chronic hyperglycemia damaging nerve fibers through multiple pathways including oxidative stress, accumulation of harmful metabolic byproducts, and impaired nerve blood supply. The resulting nerve damage progresses from the longest nerve fibers first, explaining why symptoms typically begin in the toes and feet before ascending to involve the legs and eventually the hands.
