3. Patient Responsibilities

• Arrive on time for scheduled appointments or provide adequate notice for cancellations
• Pay for services rendered in accordance with our payment policies

4. Appointments and Cancellations

Booking: Appointments can be booked online, by phone, or in person. We reserve the right to request a deposit or confirmation for certain services.

Cancellations: Please cancel or reschedule appointments at least 24 hours in advance. Late cancellations or no-shows may be subject to a cancellation fee.

Late Arrivals: Patients arriving more than 15 minutes late may need to reschedule, subject to availability.

5. Payment Terms

Payment Methods: We accept cash, credit/debit cards, and direct billing with approved insurance providers.

Insurance: If you have insurance coverage, please bring your insurance card and any required authorization. Coverage is subject to your insurance policy terms. You are responsible for any co-payments, deductibles, or services not covered by insurance.

Out-of-Pocket Payments: Payment is due at the time of service unless prior arrangements have been made. We reserve the right to refuse service for non-payment.

6. Medical Advice and Results

Diagnostic Results: Test results are typically available within 24-48 hours. Results will be provided to your referring physician and may be available through our patient portal.

Medical Advice: Information provided on our website or through our services is for informational purposes only and does not constitute medical advice. Always consult with a qualified healthcare professional for diagnosis and treatment decisions.

7. Limitation of Liability

To the maximum extent permitted by law, DCDC shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services. Our total liability shall not exceed the amount paid by you for the specific service in question.

8. Intellectual Property
