Treatment is started at low doses and gradually increased to find the optimal balance between pain relief and side effects. For refractory cases, combination therapy, TENS units, or referral to pain management specialists may be considered. Regular follow-up ensures treatment optimization.

Yes, all diabetics should have annual neuropathy screening starting at diagnosis for type 2 diabetes and five years after diagnosis for type 1 diabetes. Screening involves a foot examination assessing sensation (using monofilament and tuning fork), reflexes, and skin condition. Standard nerve conduction studies may be normal in pure small fiber neuropathy, requiring skin biopsy or specialized autonomic testing for diagnosis.
