Understanding Hypertension: Causes, Risks, and Management
Hypertension, or high blood pressure, is the most common cardiovascular condition worldwide, affecting over 1.28 billion adults globally. In the UAE, prevalence is estimated at approximately 30% of the adult population, with many cases remaining undiagnosed due to the condition's silent nature. Blood pressure is measured in millimeters of mercury (mmHg) as two numbers: systolic pressure when the heart contracts and diastolic pressure when it relaxes between beats. Normal blood pressure is below 120/80 mmHg, while consistently elevated readings above 130/80 mmHg indicate hypertension.
The vast majority of hypertension cases, roughly 90 to 95%, are classified as primary or essential hypertension, meaning no single identifiable cause is responsible. Instead, a combination of genetic predisposition, aging, dietary sodium intake, obesity, physical inactivity, alcohol consumption, and chronic stress contribute to blood pressure elevation. The remaining 5 to 10% have secondary hypertension caused by an underlying condition such as kidney disease, adrenal gland tumors, thyroid dysfunction, sleep apnea, or certain medications. Identifying secondary causes is important because treating the underlying condition can sometimes cure the hypertension.
Left uncontrolled, hypertension inflicts progressive damage on vital organs. The heart must work harder against elevated pressure, leading to left ventricular hypertrophy and eventually heart failure. Damaged blood vessels in the brain increase stroke risk. The kidneys' delicate filtering units deteriorate, leading to chronic kidney disease. Retinal blood vessels can rupture, causing vision loss. Large arteries stiffen and may develop aneurysms. These complications develop insidiously over years and are largely irreversible once established, making early detection and consistent treatment essential.
Modern hypertension management combines lifestyle modification with pharmacological treatment tailored to each patient's risk profile. The DASH diet, regular aerobic exercise, sodium restriction, weight loss, and stress management form the foundation. When medications are needed, cardiologists choose from multiple drug classes including ACE inhibitors, angiotensin receptor blockers, calcium channel blockers, diuretics, and beta blockers, often using combination therapy for optimal control. Regular follow-up with home blood pressure monitoring and periodic 24-hour ambulatory monitoring ensures sustained control and allows therapy adjustments as needed.

It detects white coat hypertension where readings are high only at doctor visits, masked hypertension where readings appear normal in the clinic but are elevated at home, and nocturnal hypertension where blood pressure fails to dip during sleep. These patterns have different treatment implications and cannot be identified by office readings alone.
When starting or adjusting medications, check your blood pressure twice daily, morning and evening, for at least one to two weeks. Take two readings each time, one minute apart, while seated quietly for five minutes beforehand. Record all readings in a log for your cardiologist. Once your blood pressure is stable and at target, monitoring two to three times per week is typically sufficient. Use a validated upper-arm monitor rather than a wrist device for best accuracy.
High blood pressure is called the silent killer because it rarely causes noticeable symptoms while silently damaging your heart, brain, kidneys, and blood vessels over years. Most people with hypertension feel perfectly normal. Symptoms typically only appear during a hypertensive crisis with readings above 180/120, presenting as severe headache, vision changes, chest pain, difficulty breathing, or nosebleeds. This is why regular blood pressure screening is essential for early detection and prevention of organ damage.
