Mammography can detect microcalcifications, small masses, and areas of asymmetric density that may indicate ductal carcinoma in situ (DCIS) or invasive breast cancer. The technology is also valuable for monitoring women with dense breast tissue, breast implants, or those undergoing treatment follow-up after previous abnormal findings.

Understanding the types of mammograms available helps you make informed screening decisions. Standard 2D digital mammography captures flat images of the breast from two angles and remains the established screening method for most women. 3D mammography, also known as digital breast tomosynthesis, takes multiple thin-slice images of the breast and reconstructs them into a three-dimensional view, reducing the overlap of tissue that can obscure findings on conventional 2D images. Tomosynthesis is particularly valuable for women with dense breast tissue, as it improves cancer detection rates while reducing false-positive callbacks that lead to unnecessary anxiety and additional testing. Your radiologist can advise which technology is most appropriate based on your breast density and risk factors.

Breast density is a critical factor in mammographic screening that many women are unaware of. Breast tissue is classified into four density categories: almost entirely fatty, scattered areas of fibroglandular density, heterogeneously dense, and extremely dense. Approximately 40 to 50 percent of women have dense breasts, and density matters for two important reasons. First, dense tissue appears white on mammograms, which can mask tumors that also appear white, reducing the sensitivity of standard mammography. Second, higher breast density is independently associated with an increased risk of developing breast cancer. For women with heterogeneously or extremely dense breasts, supplemental screening with breast ultrasound or MRI may be recommended to improve the chance of early detection. Mammogram frequency guidelines depend on your age, family history, and individual risk factors. For average-risk women, most international guidelines recommend annual screening mammograms beginning at age 40, continuing as long as overall health permits. Women with a first-degree relative diagnosed with breast cancer, known BRCA1 or BRCA2 mutations, or a history of chest radiation therapy before age 30 are considered high-risk and may benefit from starting screening at age 25 to 30, often with alternating mammography and breast MRI every six months. After age 74, screening decisions should be individualized through a conversation with your physician.
