The decision to extract wisdom teeth is based on clinical examination and imaging findings. OPG (panoramic) X-rays provide an overview of all four wisdom teeth and their relationship to surrounding structures. When greater detail is needed, particularly for lower wisdom teeth near the inferior alveolar nerve, CBCT (cone beam computed tomography) provides three-dimensional views that allow precise assessment of nerve proximity, root morphology, and bone density. This imaging-guided approach significantly reduces surgical risks and enables your oral surgeon to plan the optimal extraction technique for your specific anatomy.
The extraction procedure itself varies in complexity depending on whether the tooth is erupted, partially erupted, or fully impacted within bone. Simple extractions of erupted wisdom teeth involve loosening the tooth with an elevator instrument and removing it with forceps under local anesthesia. Surgical extractions of impacted teeth require making a small incision in the gum, sometimes removing a small amount of bone, and often sectioning the tooth into pieces for easier removal. Both approaches are performed under effective local anesthesia, and sedation options are available for patients who are anxious or have particularly complex cases.
Recovery after wisdom tooth extraction follows a predictable timeline. The first 24 hours are the most critical; patients should rest, apply ice packs to reduce swelling, and keep gauze in place as directed to protect the blood clot. Swelling peaks around day two to three and then gradually subsides over the following week. Most patients manage discomfort effectively with prescribed or over-the-counter pain medication. The most important aftercare instruction is to avoid actions that create suction in the mouth, such as using straws, smoking, or vigorous spitting, as these can dislodge the blood clot and cause a painful condition called dry socket.
