The ESR measures how fast red blood cells settle in a tube over one hour, reported in mm/hr. When inflammation is present, the liver produces proteins (fibrinogen, immunoglobulins) that cause red cells to clump into rouleaux formations, which settle faster.
Normal ESR is generally below 15 mm/hr for men under 50 and below 20 mm/hr for women under 50, increasing with age. A common formula: upper limit equals age divided by 2 for men, (age + 10) divided by 2 for women.
Mildly elevated (20-40 mm/hr): aging, obesity, or mild infections. Moderate (40-100): active rheumatoid arthritis, lupus, moderate infections, or IBD.
CRP rises within hours and falls quickly, ideal for acute monitoring and treatment response. ESR rises over days and stays elevated for weeks, better for detecting chronic inflammation.
ESR is nonspecific but has strong associations with certain diseases. In giant cell arteritis, ESR above 50 mm/hr with headache and visual symptoms strongly supports the diagnosis.
