The peripheral nervous system is a vast network of nerves extending from the brain and spinal cord to every part of the body, carrying motor commands to muscles, sensory information back to the brain, and autonomic signals that regulate involuntary functions like heart rate and digestion. When these nerves are damaged, the resulting neuropathy can produce a wide range of symptoms depending on which nerve fibers are affected. Understanding the type and distribution of nerve involvement is crucial for accurate diagnosis and targeted treatment.

Diabetic neuropathy deserves special attention in the UAE context, where diabetes prevalence is among the highest in the world. The mechanism involves chronic hyperglycemia damaging nerve fibers through multiple pathways including oxidative stress, accumulation of harmful metabolic byproducts, and impaired nerve blood supply. The resulting nerve damage progresses from the longest nerve fibers first, explaining why symptoms typically begin in the toes and feet before ascending to involve the legs and eventually the hands. This length-dependent pattern is the hallmark of diabetic polyneuropathy.

Electrodiagnostic testing with nerve conduction studies and EMG remains the gold standard for objectively assessing peripheral nerve function.
