Understanding Thyroid Disease: Causes, Diagnosis, and Treatment

The thyroid gland is a butterfly-shaped organ located at the front of the neck that produces hormones controlling metabolism, energy production, body temperature, heart rate, and many other vital functions. When the thyroid produces too little hormone (hypothyroidism) or too much (hyperthyroidism), the effects are felt throughout the entire body. Thyroid disorders are remarkably common, affecting an estimated 200 million people worldwide, with women being five to eight times more likely to develop thyroid problems than men. In the UAE, thyroid dysfunction is particularly prevalent due to factors including widespread vitamin D deficiency, dietary patterns, and genetic predisposition within certain populations.

Hypothyroidism is the most common thyroid disorder and is most frequently caused by Hashimoto thyroiditis, an autoimmune condition in which the immune system gradually damages the thyroid gland. Diagnosis relies on blood tests showing elevated TSH with low free T4 levels, often accompanied by positive anti-TPO antibodies. Treatment with levothyroxine is highly effective, but proper dosing is critical. Taking too little leaves patients symptomatic, while too much can cause hyperthyroid symptoms and increase the risk of osteoporosis and heart arrhythmias. The medication must be taken on an empty stomach, ideally 30 to 60 minutes before breakfast, and several foods and medications can interfere with absorption.
