Understanding ECG and Heart Monitoring at Home

The electrocardiogram remains the most widely used and clinically validated cardiac diagnostic tool in medicine. First developed over a century ago and continuously refined since, the 12-lead ECG provides a non-invasive, radiation-free snapshot of the heart's electrical activity that can reveal rhythm disorders, conduction abnormalities, ischaemic changes, chamber enlargement, and electrolyte imbalances. Its simplicity, speed, and diagnostic power make it the universal first step in cardiac assessment.

In Dubai, cardiovascular disease is the leading cause of mortality, accounting for a significant proportion of deaths in the UAE. Risk factors prevalent in the region including diabetes, hypertension, obesity, sedentary lifestyles, and high stress levels all contribute to accelerated cardiac risk. Early detection through regular ECG screening can identify problems before they cause symptoms, enabling preventive intervention that significantly reduces the risk of heart attack, stroke, and sudden cardiac death.

Home-based ECG recording addresses a practical barrier to cardiac screening: many patients, particularly elderly individuals and those with mobility limitations, delay or avoid cardiac testing because of the effort involved in travelling to a hospital. By eliminating this barrier, home ECG services increase diagnostic access and enable earlier intervention. Research demonstrates that ECG recordings performed in a relaxed home environment often provide a more accurate baseline than those obtained in a busy hospital setting, where white-coat anxiety can elevate heart rate and mask or mimic arrhythmias.

Continuous monitoring with Holter devices has become an essential tool for diagnosing paroxysmal arrhythmias that occur unpredictably throughout the day. Conditions such as paroxysmal atrial fibrillation, intermittent SVT, and nocturnal bradycardia may never appear on a standard 10-second ECG. A 24 to 48-hour Holter recording captures the heart's activity during sleep, exercise, meals, and emotional stress, providing the cardiologist with a comprehensive picture of cardiac behaviour across all activities of daily living.

A Holter monitor is a portable device that continuously records your heart rhythm for 24 to 48 hours while you go about your normal daily activities. It is used when a standard ECG does not capture intermittent symptoms such as occasional palpitations, brief dizzy spells, or suspected paroxysmal atrial fibrillation. Our technician fits the device at your home and returns to collect it at the end of the monitoring period. The full recording is then analysed by a cardiologist.

Every ECG recording is reviewed and interpreted by a qualified cardiologist, not by software alone. The cardiologist provides a written report including rhythm analysis, rate assessment, axis determination, interval measurements, and any abnormal findings with clinical recommendations. If an urgent abnormality is detected, the cardiologist contacts you or your referring physician immediately by phone.

Yes, an ECG can and should be performed on patients with pacemakers to verify appropriate device function and assess underlying rhythm. Our technicians are trained to identify pacemaker spikes on the tracing and the reporting cardiologist will assess capture and sensing function. However, a full pacemaker interrogation requires separate device-specific equipment and should be performed at a cardiology clinic.

No. You should take all your medications as prescribed. In fact, it is important that the ECG is recorded while you are on your usual medications, as one of the purposes may be to monitor how those medications affect your heart rhythm and intervals. Your medication list will be noted on the report so the interpreting cardiologist can account for expected drug effects.

If the cardiologist identifies an urgent finding, you will be contacted immediately by phone with guidance on next steps, which may include proceeding to a hospital emergency department or scheduling an urgent cardiology consultation. For non-urgent abnormalities, the report will include recommendations such as an echocardiogram, stress test, or cardiology review, which can be arranged at our DHCC clinic or with your own cardiologist.
